Working with Individual Service Plans
Given the rigorous state requirements for reporting on individual goals, the Therap Individual Service Plan (ISP) module provides enhanced and detailed information that allows for more methodical and concise Individual Plan-building. At its most basic, the ISP module allows tracking of an individual’s teaching skills programs as well as generating the necessary accompanying reports.

Creating an ISP begins by simply clicking on “New” under “ISP/IPP Program” which brings one to the plan’s page where the program details are entered into the system. The writer will detail the long-term objective, the goal itself, and the reason for implementing the plan. Baseline dates, criteria for completion, and tasks can be added one by one. The form itself allows for numerous options for scoring the progress of the individual.

Entering data also begins by clicking on “New” under “ISP/IPP Data” and choosing the given individual and then the given ISP. Depending on how the ISP was designed, input will vary from simply clicking on the task and choosing the appropriate score, or if within a “non-grid,” perhaps typing in the location, noting the duration of an activity, and offering commentary about the activity.

Given the flexibility and simplicity of this module, it is extremely helpful in goal-tracking for both residential and day program settings. It provides for swift report generation, saving time for the supervisory staff as well as straightforward documentation options for the DSPs.

Application Focus
Medication Error Reports

Authorized users of the Therap system are able to submit Medication Error Reports through the General Event Report application. The application is used to document a range of incidents including injury, behavior restraints and death.

The procedure is simple – clicking on the Medication Error button on the GER form opens the Medication Error Report form. On this form the users select the type of the medication error, the time it was discovered and the name of the person responsible. Users then proceed to enter the details including the name, dosage, applying frequency and other information about ordered and given medication and the total number of errors. Reporters can easily look up ordered medications by clicking on the appropriate button on the form; the information found is automatically copied into the appropriate fields. Users can report multiple medication errors on the same form this way.

In the last section of the Medication Error Report form, users can record the reason for the error. They must specify the cause of the medication error as well as whether medical attention was required due to it.

Clicking on the Add button at the bottom end of the form includes the medication error data in the General Event Report. Users then complete the rest of the GER form and submit it for approval.

How do administrators certify users using the Training Management System (TMS)?

When a staff passes a training curriculum, Training Administrators are able to certify them simply by selecting the Certify User(s) link under 'Training Class Certification'. Clicking on Certify User(s) link will take them to a list of all training classes at the agency. When a class is selected, it will show a list of all staff who were assigned to that class. The administrator then goes through this list and certifies the people who have passed the curriculum by simply selecting them on the list.
